Attention and Behavioral Issues

You have contacted our office regarding behavioral and/or learning concerns about your child. A common concern for parents and/or teachers is whether a child is having significant difficulty sustaining focus on activities. These attention difficulties can interfere with performance and behavior at home and at school. If you would like to have your child screened for attention difficulties, please download and complete the following forms and then make an appointment for us to review them together. National guidelines require your child to be between the age of 6 and 12 years old in order for us to evaluate for attention difficulties. So that we may provide the best possible care for your child, please download and forward the following to the office before your appointment:

Please have all pertinent information forwarded to the office. Once the provider has reviewed all information, we will contact you to arrange an appointment. At your appointment, we will review these materials and recommendations for further treatment or evaluation will be discussed.

In addition, please provide copies of any previous evaluations done at school or by any other health professional. A copy of your child's most current report card and any TAKS or achievement test scores would also be very helpful. At your appointment, you will meet with the provider to review the assessment forms and potential diagnoses. Please call if you have any questions.
